For decades, The ASAM Criteria has been the shared language of addiction treatment, guiding how providers assess patients, recommend a level of care, and plan treatment, continued stay, and discharge. In 2023, the American Society of Addiction Medicine (ASAM) released the Fourth Edition, its first major update in more than ten years. With payers increasingly using the Fourth Edition for medical necessity reviews, providers need more than familiarity with the new standards. They need documentation tools built around them.
What’s New in the Fourth Edition
Updated six dimensions. The dimensions were renamed and reordered to reflect current clinical language:
- Intoxication, Withdrawal, and Addiction Medications
- Biomedical Conditions
- Psychiatric and Cognitive Conditions
- Substance Use-Related Risks
- Recovery Environment Interactions
- Person-Centered Considerations
Readiness to change is no longer its own dimension. It is now considered across all dimensions, and the new Dimension 6 uses shared decision-making to determine the level of care a patient is willing and able to engage in.
Subdimensions. Each dimension now includes subdimensions describing the specific, actionable factors clinicians should assess, giving assessments more structure and consistency.
A refined continuum of care. The four broad levels (outpatient, intensive outpatient/high-intensity outpatient, residential, and inpatient) remain, but with clearer numbering and naming. Notable changes include a new Level 1.0 for long-term remission monitoring, the removal of Level 0.5, and the option to recommend a recovery residence alongside outpatient care.
Integrated care at every level. All levels of care are now expected to be co-occurring capable. Withdrawal management, cognitive impairment, and harm reduction are built into every level instead of being treated separately.
No more risk ratings. The Third Edition’s dimensional risk ratings have been replaced by a new dimensional admission criteria framework.
Two distinct assessments. The Fourth Edition sets separate standards for the Level of Care Assessment and the Treatment Planning Assessment.
Why ASAM 4 Belongs Inside Your EHR
The Fourth Edition is more structured, more person-centered, and more detailed than its predecessor. That’s good for patients, but it raises the bar for documentation. Payers expect the reasoning behind a level-of-care recommendation to be clearly documented across every dimension, not just a diagnosis and a level.
Most general-purpose EHRs were never designed with ASAM as a core workflow. Clinicians end up forcing generic templates to fit, which leads to:
- Incomplete or inconsistent dimensional documentation
- Denied or delayed authorizations
- Extra time spent on paperwork instead of patient care
- Difficulty tracking level-of-care changes over the course of treatment
An EHR with ASAM 4 embedded guides clinicians through each dimension and subdimension, keeps assessments consistent across your team, and produces documentation that stands up to utilization review.
How MedEZ Supports the ASAM Criteria
MedEZ has served behavioral health providers for more than 30 years, and ASAM support is built directly into the application, not added on.
- ASAM 4th and 3rd Edition embedded. Use the Fourth Edition where payers require it, while keeping the Third Edition available where it is still in use (for example, some payers continue to apply it for adolescent reviews).
- CO-Triage and the Comprehensive Assessment. Both ASAM 4 tools are available inside MedEZ, from initial screening through full level-of-care assessment.
- Connected to the full clinical record. ASAM assessments work alongside the Wiley Treatment Planner, Utilization Review, Admissions & Discharge Workflows, and Billing & Revenue Cycle Management, so placement decisions flow straight into treatment plans and authorizations.
- Inpatient and outpatient ready. MedEZ supports programs across the continuum of care.
Ready for ASAM 4?
The shift to the Fourth Edition is already underway across many payers. Make sure your documentation is ready.
